Corporate Customer
Account owner as subscriber

• A certified true copy the Committee(s) identification cards and certificate of incorporation.

For customers who have been with AIS for less than 3 months

• Additional document of corporate's bank passbook or a copy of corporate's cash flow statement covering
  previous month

Authorized contact person as subscriber

• A certified true copy of authorized person's identification card in addition to the required documents above

In the case of authorization

• A certified true copy of the authorized person (s) identification card - A certified true copy of the attorney's
  identification card
