receive coverage rights
A :
To apply for the insurance and get the coverage, the customer is
required to press the USSD from the number within
15 days
after the SIM activation day. Failure to do this shall
be deemed as a waiver of the customer’s right. After registration to get
the right, the company shall send an SMS confirmation message within one
day.
Q :How Long
Is the Insurance coverage Period?
A :
The policy period is maximum up to 45 days (including 14 days waiting
period) and will be effective once you received the confirmation message
from the company.
Q :When is
the insurance effective and expiry date?
A :
The insurance will coverage will effective once you received the
confirmation message from the company and will expire in the next 45
days (including 14 days waiting period) at 24.00 hrs. (Local Time)
Q :Covid-19
infection insurance coverage (Coronavirus (2019-nCoV))
A :
This policy covers Critical illness or coma caused by the Coronavirus
Infection* / Infection due to COVID-19 (First Infected Only) / and Loss
of Life, Dismemberment, Loss of Sight or Total Permanent Disability
(PA.1) as the specified sum insured in the policy.
Q :
How old must the insured be to receive the Covid-19 insurance?
A :
The insured aged between 7 year - 99 years (current year minus the year
of birth) as of the registration date via USSD to get the insurance.
Q :Have the
customer need health check-up before to get health insurance?
A :
The health check-up is not necessary for this program. However, the
customer should have a good health without having any disability and
chronic disease.
Q :Can the
customer choose the effective date of the insurance?
A :
The effective date will be the date received the confirmation message
automatically.
Q :If I
activate new SIM more than one SIMs, can I register for a coverage for
other individuals
A :
The company reserves the right to provide coverage to one customer per
right per new SIM per insurance policy (1 right/person). In case the
customer activates more than one new participating SIMs, the customer
shall get the coverage for the first SIM used to press the USSD code for
registration only.
Q :Will the
policy still be effective after I cancel the service or move to a
different network
A :
The company reserves the right to provide the coverage to AIS customers
only. Should a customer cancel the service or move to a different
network, the coverage shall automatically be deemed terminated without
prior notice.

Claims reimbursement for Critical illness or coma caused by the
Coronavirus Infection*
A :
The insured or the beneficiary must submit following for claim
reimbursement as soon as possible or within 30 days after being Critical
illness or coma caused by the Coronavirus Infection*
- Completed Claim Form
- Medical Report or Medical Certificate which specifies cause,
treatment period
- Laboratory test results
- Copy of Insured’s National ID Card or Passport
- Copy of Bank Book (Saving Account)
- Other documents requested by the insurance company (if any)

A :

reimbursement as soon as possible or within 30 days after being Infected
due to COVID-19 (First Infected Only)

Total Permanent Disability (PA.1)
A :
1. Claims reimbursement for Loss of Life
The beneficiary must submit following documents as soon as possible or
within 30 days in case of loss of life.
- Completed Claim Form
- Medical Report or Medical Certificate which specifies cause,
treatment period.
- Certified true copy of Death Certificate issued by District
- Certified true copy of Death Certificate issued by the hospital,
in case of being hospitalized before passing away) and all
related medical history
- Certified true copy of Death Certificate issued by Royal Thai
Police, in case of death at accident scene
- Certified true copy of Autopsy Report from the local police
station where the accident occurred
- Certified true copy of Police Report from the local police
station where the accident occurred and the Investigation Report
- Certified true copy of Passport of the Insured
- Certified true copy of Work Permit for foreign worker
- Certified true copy of Insurance Certificate
- Certified true copy of Passport or ID card of the Beneficiary
- Certified true copy of Bank Book of the Beneficiary
- Other additional documentation is depending on the loss
circumstances and necessity
2. Claims reimbursement for Dismemberment, Loss of Sight or Total
Permanent Disability
The insured or the beneficiary must submit following documents as soon
as possible or within 30 days in case of Dismemberment, Loss of Sight or
Total Permanent Disability.
- Completed Claim Form
- Medical Report or Medical Certificate which specifies cause,
treatment period.
- Medical Certificate confirming the injury, dismemberment or
Total Permanent Disability
- Certified true copy of Police Report from the local police
station where the accident occurred and the Investigation Report
- Certified true copy of Passport of the Insured
- Certified true copy of Work Permit for foreign worker
- Certified true copy of Insurance Certificate
- Other additional documentation is depending on the loss
circumstances and necessity
